✔️ How does Oxifyl CR 400 work?

Pentoxifylline enhances blood flow properties by decreasing blood viscosity and improving erythrocyte flexibility. This results in better oxygen delivery to tissues in patients with chronic peripheral arterial disease. The exact mechanism leading to clinical improvement is not fully understood. Pentoxifylline has been shown to:

• Improve leukocyte deformability.
• Inhibit neutrophil adhesion and activation.
• Increase tissue oxygen levels at therapeutic doses.
  It achieves dose-dependent hemorrheologic effects, reducing blood viscosity and improving microcirculation.

✔️ Commonly reported adverse effects:

• Cardiovascular: Arrhythmia, tachycardia, angina pectoris.
• Hematological: Thrombocytopenia, leukopenia/neutropenia.
• Nervous System: Dizziness, headache, aseptic meningitis.
• Gastrointestinal: Nausea, vomiting, diarrhea, epigastric discomfort, constipation, hypersalivation.
• Skin: Pruritus, rash, urticaria, erythema.
• Vascular: Flushing, bleeding.

• Dosage should be adjusted based on the severity of the condition and patient tolerance.
• Hepatic Impairment: Dose reduction required, based on tolerance.
• Renal Impairment: Reduce dose by 30-50% for creatinine clearance <30 mL/min.
• Hypotensive or High-Risk Patients: Start at low doses and increase gradually.

• Diabetes Treatment: Pentoxifylline may enhance the blood-sugar-lowering effect of insulin or oral antidiabetic agents. Monitor blood glucose levels carefully.
• Anticoagulants: Increased anticoagulant activity may occur when used with anti-vitamin K. Monitor coagulation levels when starting or adjusting the dose of Oxifyl CR.
• Antihypertensives: May potentiate blood pressure-lowering effects.
• Theophylline: May lead to elevated theophylline levels, increasing adverse reactions.
• Ciprofloxacin: May increase the serum concentration of Oxifyl CR, intensifying side effects.
• Platelet Aggregation Inhibitors: Concomitant use increases bleeding risks. Use with caution.
• Cimetidine: May raise plasma levels of Oxifyl CR and its active metabolite.

• Hypersensitivity to pentoxifylline, methylxanthines, or excipients.
• Patients with severe or extensive bleeding (e.g., massive or retinal bleeding).

• Pregnancy: Insufficient data is available; avoid use unless necessary.
• Lactation: Minute quantities of pentoxifylline pass into breast milk. Use only if the potential benefit outweighs the risk.

✔️ Precautions & Warnings

• Discontinue immediately at the first sign of an anaphylactic reaction.
• Monitor closely in:
  • Patients with severe cardiac arrhythmias or recent myocardial infarction.
  • Hypotensive patients or those with unstable circulation.
  • Renal impairment (creatinine clearance <30 mL/min).
  • Severely impaired liver function.
  • Patients with increased bleeding risk or concurrent use of anticoagulants/platelet inhibitors.
  • Patients on ciprofloxacin or theophylline.
