✔️ Dosage & Administration of Insimet 850

Immediate Release Tablet

Adults:

• Usual starting dose: 500 mg twice daily or 850 mg once daily with meals
• Dose may be increased gradually by 500 mg weekly or 850 mg every two weeks according to response
• Maximum recommended dose: 2000 mg daily; in some patients up to 2550 mg daily in divided doses

Children:

• Initial dose: 500 mg twice daily with meals
• Dose may be increased gradually up to 2000 mg daily in divided doses

Extended-Release Tablet

• Swallow whole without crushing or chewing

Adults:

• Starting dose: 500 mg once daily with the evening meal
• Dose may be increased weekly by 500 mg, depending on glycemic control
• Maximum dose: 2000 mg daily

Patients taking immediate-release Metformin may switch to the extended-release formulation while maintaining the equivalent total daily dose.

Renal Impairment

• Contraindicated if eGFR is below 30 mL/min/1.73 m²
• Evaluate risks and benefits if eGFR falls below 45 mL/min/1.73 m²

Elderly Patients

Dose adjustment may be required due to age-related decline in kidney function.

Pediatric Use

Type 2 diabetes diagnosis should be confirmed before starting treatment in children. Growth and development should be monitored during therapy.

✔️ Interaction

• Nifedipine may increase Metformin absorption
• Cationic drugs such as Digoxin, Quinidine, Vancomycin, and Trimethoprim may interfere with renal elimination of Metformin
• Certain medicines like corticosteroids, oral contraceptives, diuretics, thyroid hormones, calcium channel blockers, and phenytoin may increase blood sugar levels and reduce glycemic control
• Reduced renal clearance may occur with cimetidine

• Nausea
• Vomiting
• Diarrhea
• Abdominal discomfort
• Loss of appetite
• Taste disturbance
• Lactic acidosis
• Vitamin B12 deficiency
• Hepatitis or abnormal liver function tests
• Skin reactions such as rash, itching, or urticaria
• Hemolytic anemia

✔️ Insimet should not be used in patients with:

• Hypersensitivity to Metformin or any component of the formulation
• Severe renal impairment (GFR below 30 mL/min)
• Diabetic ketoacidosis or metabolic acidosis
• Severe dehydration, infection, or shock
• Cardiac or respiratory failure
• Recent myocardial infarction
• Hepatic impairment or alcoholism

✔️ Pregnancy & Lactation

Pregnancy

Uncontrolled diabetes during pregnancy increases the risk of fetal complications. Insulin is generally preferred during pregnancy to maintain blood glucose control.

Breastfeeding

Metformin passes into breast milk in small amounts. Breastfeeding during treatment is generally not recommended unless advised by a physician.

✔️ Precautions & Warnings

• Monitor renal function regularly as Metformin is excreted through the kidneys
• Risk of lactic acidosis increases in renal impairment
• Long-term use may lower Vitamin B12 levels
• Temporary discontinuation may be required before surgery or radiologic investigations using contrast media
• Use cautiously in elderly patients and those with dehydration or severe infections
• Monitor blood glucose carefully when combined with insulin or sulfonylureas due to the risk of hypoglycemia
