✔️ How does Finen 10mg Tablet work?

Finerenone blocks aldosterone, a hormone that regulates salt and water balance. This leads to lower blood pressure and relieves strain on the kidneys. It can slow down kidney damage in people with diabetes and chronic kidney disease.

✔️ Side Effects of Finen 10mg Tablet

• Hyperkalaemia (High potassium level)
• Hyponatraemia (Low sodium level)
• Hypotension (Low blood pressure)
• Hyperuricaemia (High uric acid level)
• Pruritus (Itching)

✔️ Quick Suggestions:

• Avoid breastfeeding at the time of treatment with Finerenone and for one day after treatment.
• For patients who are unable to swallow whole tablets, Finerenone may be crushed and mixed with water or soft foods, before taking it.
• Inform your doctor if you are taking potassium supplements or any other medications for high blood pressure or heart disease.
• Women of childbearing potential should use effective contraception during the treatment to avoid getting pregnant.
• Finerenone contains lactose; inform your doctor if you have lactose intolerance.

✔️ How to take Finen 10mg Tablet

Take Finen 10mg Tablet as directed by your physician. Swallow the medicine with a glass of water. Do not crush or chew the medicine. It can be taken with or without meals.

✔️ Mechanism of Action:

• Finerenone is a nonsteroidal, selective antagonist of the mineralocorticoid receptor (MR).
• It blocks MR-mediated sodium reabsorption and MR overactivation in both epithelial (e.g., kidney) and nonepithelial (e.g., heart, blood vessels) tissues, which can contribute to fibrosis and inflammation.
• Finerenone has high potency and selectivity for MR, with no significant affinity for androgen, progesterone, estrogen, or glucocorticoid receptors.

✔️ Pharmacokinetics:

• Absorption: Finerenone is completely absorbed after oral administration, with an absolute bioavailability of 44%. Peak plasma concentration (Cmax) is achieved within 0.5 to 1.25 hours after dosing.
• Distribution: Volume of distribution (Vss) is 52.6 L. Plasma protein binding is 92%, mainly to serum albumin.
• Elimination: Terminal half-life is 2 to 3 hours. Systemic blood clearance is about 25 L/h.
• Metabolism: Primarily metabolized by CYP3A4 (90%) and to a lesser extent by CYP2C8 (10%) to inactive metabolites. About 80% of the dose is excreted in urine.

✔️ Dosage & Administration:

• Starting Dose:
  • eGFR ≥60 mL/min/1.73 m²: 20 mg once daily
  • eGFR ≥25 to <60 mL/min/1.73 m²: 10 mg once daily
  • eGFR <25 mL/min/1.73 m²: Not recommended

✔️ Dose Adjustment:

Increase dosage to 20 mg once daily after 4 weeks based on eGFR and serum potassium levels.

✔️ Missed Dose:

• Take as soon as possible on the same day. If missed for a day, skip and resume the next dose as scheduled.

✔️ Administration:

Tablets can be taken with or without food. If swallowing is difficult, tablets can be crushed and mixed with water or soft foods.

✔️ Geriatric Use:

No dose adjustment is required based on the study data.

✔️ Pediatric Use:

• Safety and efficacy are not established for those under 18.

✔️ Hepatic Impairment:

Avoid severe hepatic impairment. No adjustment is needed for mild/moderate impairment but monitor potassium levels.

✔️ Overdose:

• Symptoms: Likely hyperkalemia.
• Treatment: Discontinue Finerenone. Standard treatment for hyperkalemia. Finerenone is not efficiently removed by hemodialysis.

✔️ Interactions:

• Strong CYP3A4 Inhibitors: Contraindicated (e.g., itraconazole, ketoconazole, ritonavir).
• Moderate/Weak CYP3A4 Inhibitors: Monitor serum potassium and adjust the dose as needed.
• Strong/Moderate CYP3A4 Inducers: Avoid use (e.g., rifampicin).
• Drug-Food Interactions: Avoid grapefruit or grapefruit juice.

• Strong CYP3A4 inhibitors
• Adrenal insufficiency

✔️ Pregnancy & Lactation

• Pregnancy: No data on risks. Animal studies show developmental toxicity. Avoid unless necessary.
• Lactation: Likely present in breast milk. Avoid breastfeeding during and 1 day after treatment.

✔️ Precautions:

• Monitor serum potassium and eGFR before and during treatment.
• Do not start treatment if serum potassium >5.0 mEq/L.
