✔️ Pharmacology

Gentamicin is an aminoglycoside that binds to 30s and 50s ribosomal subunits of susceptible bacteria disrupting protein synthesis, thus rendering the bacterial cell membrane defective.

✔️ Dosage & Administration of Gento-HC Cream

Adult Dose

• Adult: Apply to the affected area 3-4 times daily.

Child Dose

• Child> 1 year: Apply to the affected area 3-4 times daily.

✔️ Interaction

Gentamycin: additive effect with other neurotoxic and/or nephrotoxic drugs, including cephalosporins, methicillin, amphotericin B, cyclosporin, cisplatin, powerful diuretic (for example, etc. Ecstrist acid. , furosemide) and neuromuscular blocking agents (eg succinylcholine, tubocurarine). It can potentiate the effect of anticoagulants (for example, Warfarin, Feninnione). It can antagonize the effect of neostigmine and pyridostigmine. Increased risk of hypocalcemia with bisphosphonates. Increased risk of neuromuscular blocking with botulinum toxin.  Indomethacin can increase the plasma concentration of gentamicin in newborns.

Hydrocortisone: Tiazides can improve hyperglycemia and hypokalemia caused by corticosteroids. Higher incidence of peptic ulcer or GL bleeding with simultaneous many says. Response to modified anticoagulants. The dose of antidiabetic and antihypertensive must be increased.  Decreases serum salicylates and antimuscarinic agents. Ethanol can improve the irritation of the gastric mucosa. Reduction efficiency with concomitant use of carbamazepine, phenytoin, primidone, barbiturates, and rifampicin. The mutual inhibition of metabolism between cyclosporin and corticosteroids increases the plasma cone of the two drugs. An improved effect on women taking estrogen of prayer or contraceptives. 

✔️ Contraindications

History of hypersensitivity to aminoglycosides; pregnancy; Hepatic insufficiency.

✔️ Pregnancy & Lactation

Animal reproduction studies have shown adverse effects on the fetus and there are no adequate and well-controlled studies in humans, but the potential benefits may justify its use in patients pregnant women despite the potential risks.

✔️ Precautions & Warnings

Concomitant use of neuromuscular blocking agents; myasthenia gravis, parkinsonism; conditions predisposing to ototoxicity and nephrotoxicity; Breastfeeding. Monitor plasma gentamicin concentrations in patients receiving high doses or prolonged therapy, neonates, the elderly, patients with renal impairment, cystic fibrosis, or severe obesity. Monitor hearing and kidney function.
