Vitamin D3, when combined with calcium, is utilized to both prevent and treat bone diseases like rickets (characterized by softening and weakening of bones in children due to vitamin D deficiency), osteomalacia (softening and weakening of bones in adults due to vitamin D deficiency), and osteoporosis (a condition marked by thinning and weakening of bones, making them more prone to fractures).

Pharmacology

Cholecalciferol (Vitamin D3) helps in the absorption and reabsorption of Calcium and Phosphorus. Vitamin D3 is essential for normal bone growth and to maintain bone density. It also reduces the severity of bacterial infection, improves lung function, prevents the risk of cancer (breast, colorectal), and helps to maintain adequate insulin levels for type 2 diabetes patients.

Dosage & Administration of Delight

Adult Dose

Oral/IM:

• Nutritional deficiency: 5-10 mcg or 1-2 ml (200-400 IU) per day
• Osteoporosis prophylaxis and treatment in individuals over 50 years: 800-1000 IU (20-25 mcg) orally once daily with calcium supplements. May also be administered via IM injection.
• Hypoparathyroidism: 50,000-200,000 IU (0.625-5 mg) orally once daily with calcium supplements. May also be administered via IM injection.

Child Dose

• Nutritional Supplementation:
  • 0-12 months: 400 IU (10 mcg) orally once daily
  • 1-18 years: 600 IU (15 mcg) orally once daily
• Vitamin D-Resistant Rickets: 12,000-500,000 IU (0.3-12.5 mg) orally once daily
• Familial Hypophosphatemia: 40,000-80,000 IU (1-2 mg) orally once daily with phosphate supplements; may be reduced after the stage of growth is complete.

Overdose Effects of Delight 200000 IU/ml

It can lead to hypervitaminosis D.

Interaction

Cholecalciferol may interact with various medications, including:

• Thiazide diuretics, calcium, or phosphate (increased risk of hypercalcemia)
• Antiepileptics (e.g., carbamazepine, phenobarbitone, phenytoin & primidone) may increase vitamin D requirements.
• Rifampicin & isoniazid may reduce the efficacy of vitamin D.
• Corticosteroids may counteract the effect of vitamin D.
• Digoxin or any cardiac glycoside.
• Reduced absorption when taken with cholestyramine, colestipol, mineral oil, orlistat.
• Ketoconazole.

Contraindications

It is contraindicated in patients with known hypersensitivity to Vitamin D3.

Pregnancy & Lactation

Studies have shown safe use of doses up to 4000 IU during pregnancy. Vitamin D3 and its metabolites are excreted in breast milk. Overdose in infants induced by nursing mothers has not been observed; however, when prescribing additional vitamin D3 to a breastfed child, the practitioner should consider the dose of any additional vitamin D3 given to the mother.

Precautions & Warnings

• Caution should be exercised in individuals with the following conditions:
• High blood calcium or phosphorus levels
• Heart problems
• Kidney disease
• Vitamin D must be taken with adequate amounts of both calcium and magnesium supplementation.
• Lactation: Colecalciferol is distributed into breast milk; use with caution.
