How does Thyrolar 50mcg Tablet work?

Thyrolar 50 is a synthetic version of the thyroid hormone. It replaces the hormones that the thyroid gland cannot produce in sufficient quantity, relieving symptoms of hypothyroidism.

Indications:

Thyrolar 50mcg Tablet is used to treat hypothyroidism (underactive thyroid gland) by replacing the insufficiently produced thyroid hormones and regulating the body's energy and metabolism. It may also be prescribed for TSH suppression and myxoedema coma.

Pharmacology

• The tablet contains synthetic Levothyroxine (Thyroxine or T4), identical to the natural hormone T4 produced in the Thyroid gland.
• Approximately 30% of T4 is converted to the more active Triiodothyronine (T3) in peripheral tissues.
• TBG (Thyroxine Binding Globulin) is the major carrier of T4, protecting it from metabolism and resulting in a long half-life in circulation.
• About 0.03% of total T4 in plasma is unbound.
• The half-life of elimination of T4 is 6 to 7 days, shorter in hyperthyroidism (3 or 4 days) and longer in hypothyroidism (9 to 10 days).
• Conditions like nephrosis or hepatic cirrhosis, or certain drugs inhibiting protein binding, may shorten the half-life of T4.
• The liver is the major site of degradation of Thyroid hormones.
• T4 is conjugated with Glucuronic and Sulphate conjugates through the Phenolic hydroxyl group and excreted in the urine.
• There is an enterohepatic circulation of the Thyroid hormones, as they are liberated by hydrolysis in the intestine and reabsorbed.
• Due to the long half-life of T4, a steady blood level of the more active T3 can be achieved with a single daily dose of Levothyroxine.
• Once the correct dosage is established, variations in the therapeutic effect are unlikely.

Dosage & Administration of Thyrolar 50mcg Tablet

Dosage:

• The initial dose is determined by the doctor based on blood test results.
• Regular blood tests are conducted to assess effectiveness, and the dose may be adjusted accordingly.

Administration:

• Take on an empty stomach, ideally in the morning before the first meal.
• Avoid food, milk, or tea 1 hour before and 2 hours after taking the medicine.

Interaction

• Reduced absorption with certain medications.
• Interactions with drugs like amiodarone, propranolol, carbamazepine, phenytoin, phenobarbital, rifampicin, lithium, and others.
• Potential alteration of antidiabetic drug requirements.
• Increased risk of hypertension and tachycardia with ketamine.
• May affect the anticoagulant effect of warfarin.

• Recent myocardial infarction

Pregnancy & Lactation

Pregnancy Category A. Pregnancy may increase Levothyroxine requirements. Although Thyroid hormones are excreted only minimally in human milk,caution should be exercised when it is administered to a nursing woman.However, adequate replacement doses of Levothyroxine are generally needed to maintain normal lactation.
