Pharmacology

This is an oral contraceptive tablet containing both the synthetic progestin drospirenone and the synthetic estrogen ethinyl estradiol. This tablet's contraceptive impact is dependent on the interplay of several variables, the most significant of which are ovulation inhibition and changes in cervical secretions. If the tablet is taken as directed, it will prevent the eggs from developing to the point where they can be fertilized, and the cervical mucous will stay viscous, creating a sperm barrier and preventing the endometrium from accepting implantation. The estrogen/progestin combo has several beneficial qualities aside from stopping pregnancy. It can be used to identify contraceptive techniques, in addition to their bad qualities. Periods are more frequent, menstruation is less unpleasant, and blood is lighter. 

Drospirenone has anti-mineralocorticoid action and can prevent estrogen-induced sodium retention. Drospirenone has a positive lipid profile when combined with ethinyl estradiol, with a rise in HDL high-density lipoproteins. Drospirenone has antiandrogenic action but has no effect on the rise in ethinyl estradiol-related sex hormone binding globulin (SHBG), which can bind and inactivate endogenous androgens. When coupled with anti-mineralocorticoid and anti-androgenic qualities, drospirenone has biochemical and pharmacological properties that are very comparable to the endogenous hormone progesterone.

Dosage of Rosen 28

Adult Dosage: 

  • 1 active tablet (3 mg drospirenone/0.03 mg EE) PO every day for 21 days for Contraception
  • Then 1 inactive tablet PO every day for 7 days and 1 active tablet (3 mg drospirenone/0.02 mg EE) PO every 24 hours for 24 days
  • Then 1 inactive tablet PO every 4 days Dysphoric Premenstrual Syndrome Premenstrual dysphoric disorder (PMDD) symptoms, but only if oral contraception is used for birth control.
  • 1 active tablet (3 mg drospirenone/0.02 mg EE) PO every 24 hours for 24 days
  • Then 1 inactive tablet PO every 4 days Avoid use if you have a liver condition.

Dose for Children

14 years of contraception: 

  • Safety and effectiveness unknown >14-year-olds receive the same dose as adults.

The Renal Dose

Take Rosen 28 precisely as prescribed by your doctor. Rosen 28 should be taken with a drink of water. Do not eat or pulverize the medication. Rosen 28 can be used with or without food. Take Rosen 28 at the same hour every day if possible.

Administrations of Rosen 28

When to consume the tablets and how to ingest them:

  • This tablet box includes 21 tablets. Each pill is labeled on the package with the day of the week it should be consumed. Take your pill around the same time every day, with some water if required. Continue in the direction of the lines until all 21 pills have been consumed. Take no tablets for the next 7 days. Menstruation (the withdrawal bleed) should commence during these 7 days; rarely, it will begin on days 2-3 after the last tablet. Even if your menstruation persists, begin taking your next pack on the eighth day. This means you'll always start fresh packets on the same day of the week, and your withdrawal bleed will occur on roughly the same days each month.

Starting your first pack of this tablet: 

  • If you haven't used a hormonal control in the previous month. On the first day of your period, or the first day of menstrual bleeding, start consuming this pill. Take a notepad that has that weekday written on it. Take a pill with the label Sunday, for instance, if your menstruation begins on a Sunday. the days in that sequence, then. You may also begin between days 2 and 5 of your cycle, but in that case, you must also use a second means of contraception (the barrier technique) for the first seven days of the first cycle of taking the tablets.

After switching to another combination pill: 

  • The day following the day you finish the last tablet from your current Pillbox, you can begin consuming this tablet. (this means no tablet-free break). You can begin taking this medication the day after the first active tablet if your current pill package also includes inert pills. (if you are not sure which this is, ask your doctor or pharmacist). You can begin later, but never later than the day after your current medication's pill break. (or the day after the last inactive tablet of your present Pill).

When switching from a progestogen-only (Mini Pill) pill: 

  • You can cease taking the mini-pill at any moment and begin taking this tablet at the same time the following day. But make sure you also use a second method of contraception (a barrier technique) for the first seven days after starting the medication.

When switching from an injectable or implant: 

  • Begin taking this pill on the day your device is removed or when your next injection is scheduled. But for the first seven days after taking the tablet, make sure you also use a second means of contraception (a barrier technique).

After giving Birth:

Interaction
  • HIV medications (atazanavir/cobicistat, ospemifene, tamoxifen, and tizanidine), medications for the treatment of chronic hepatitis C (ombitasvir/paritaprevir, and ritonavir), medications for the treatment of hypertension (losartan, valsartan, and enalapril), and analgesics may interfere with Yasmin Tablet 21. (Ibuprofen, naproxen).
    Interactions between drugs and diseases include those involving hyperkalemia, irregular vaginal hemorrhage, cancer, hypercalcemia (increased calcium level in breast cancer), and hypertension. Yasmin Tablet 21 may also combine with other drugs. (high blood pressure).
  • It is advised that a non-hormonal back-up means of contraception be used in addition to the usual consumption of Rosen 28 when Rosen 28 is used concurrently with other medications that may cause reduced ethinylestradiol serum concentrations.
Contraindications

In the event of any of the following circumstances, preparations having estrogen/progestogen combinations should not be used. The use of the product should be halted right away if any of the symptoms manifest for the first time while being used.

  • Signs of an ocular blood clot (such as sudden loss of vision or painless blurring of vision)
  • Cardiac attack warning signs (chest pain, pressure, heaviness in the chest, abnormal heartbeat, extreme weakness, anxiety, or shortness of breath, sweating, nausea, vomiting, or dizziness)
  • Stroke warning signs (sudden weakness or numbness of the face, arm, or legs, especially on one side of the body, confusion, trouble in speaking or understanding, walking, dizziness, loss of balance, fainting with or without seizure)
  • Intense stomach discomfort, swelling, and a small amount of blue discoloration on a limb
  • Depressed and considering hurting or murdering yourself
  • (Arterial or Venous) thrombosis, either current or past (e.g. deep venous thrombosis, pulmonary embolism, myocardial infarction, cerebrovascular accident)
  • The presence or previous incidence of thrombus prodromi (e.g.transient ischaemic attack, angina pectoris)
  • Migraines with localized neurological complaints, present or past
    diabetic arterial involvement in addition to diabetes
  • Disturbed Lipometabolism
  • A contraindication could also be the existence of serious or numerous risk factors for venous or arterial thromboembolism.
  • Pancreatitis, or a history of it, if it is brought on by extremely high triglycerides
  • Uterine hemorrhaging that is undiagnosed
  • Existing or suspected pregnancy

Pregnancy & Lactation

Use During Pregnancy: 

  • Rosen 28 delivery is not advised while pregnant. A woman must rule out pregnancy before beginning Rosen 28. Rosen 28 needs to stop being used right away if conception develops while taking it.
    There is no evidence that oral contraceptives harm the developing fetus or raise the risk of abortion in women who stop using them before getting pregnant. However, it is advised to use a non-hormonal means of contraception for three months before trying to procreate in women who stop using oral contraceptives with the intention of getting pregnant.

Utilization During Breastfeeding:
