Common Side Effects of Repace H (12.5mg+50mg) Tablet

1. Cough
2. Respiratory problems (infections in the upper airway, nasal congestion, or sinusitis)
3. Diarrhea, stomach pain, nausea, indigestion
4. Muscle pain or cramps
5. Pain in legs and back
6. Lack of sleep (insomnia)
7. Headache
8. Dizziness
9. Tiredness or weakness
10. Chest pain
11. Signs of increased potassium levels (muscle weakness, irregular heartbeat, chest pain)
12. Altered kidney function
13. Signs of low blood sugar (dizziness, shaking, sweating, tiredness, and fast heartbeat)

Rare Side Effects (Seek Immediate Medical Attention):

• Severe allergic reactions (skin rash, itching, swelling of the face, lips, mouth, or throat, difficulty in swallowing or breathing)
• Kidney failure
• Bleeding or bruising, repeated infections (due to low white blood cells)
• Heart problems (abnormal heartbeat, cerebrovascular accident or heart attack)
• Fluid accumulation in the lungs
• Pancreatitis (inflammation of the pancreas resulting in severe pain in the back and abdomen)
• Toxic epidermal necrolysis (skin rash with blisters and skin peeling)
• Hepatitis (inflammation of the liver)

Uncommon Side Effects:

1. Anemia
2. Red or brownish spots on the skin
3. Clotting problems
4. Loss of appetite
5. Gout (increased uric acid levels)
6. Signs of increased blood sugar levels (increased thirst, blurred vision, weakness)
7. Abnormal blood electrolyte levels
8. Anxiety, nervousness, panic disorder
9. Confusion, depression, abnormal dreams
10. Sleep disorders, sleepiness, memory impairment
11. Tingling or prickling sensation in arms, hands, legs, or feet
12. Trembling, migraine, fainting
13. Blurred vision, burning or stinging in the eyes, conjunctivitis
14. Worsening eyesight, seeing things in yellow
15. Ringing, buzzing, roaring, or clicking in the ears
16. Vertigo (spinning sensation)
17. Low blood pressure associated with postural changes (feeling light-headed or weak when you stand up)
18. Inflammation of blood vessels (associated with skin rash or bleeding)
19. Sore throat, breathlessness, bronchitis, pneumonia
20. Nosebleed, runny nose, congestion
21. Constipation, obstipation
22. Wind, stomach upsets, stomach spasms, vomiting, dry mouth
23. Inflammation of a salivary gland, toothache
24. Jaundice (yellowing of the eyes and skin)
25. Itching, inflammation of the skin, rash, redness of the skin, sensitivity to light, dry skin, flushing, sweating, hair loss
26. Joint swelling, stiffness, muscle weakness
27. Frequent urination including at night, urinary infection
28. Sugar in urine (glycosuria)
29. Decreased sexual appetite, impotence
30. Swelling of the face, localized swelling (edema)
31. Fever

How to Manage Side Effects:

Nausea:

• Take Repace H (12.5mg+50mg) Tablet with or just after meals.
• Stick to simple meals and avoid rich or spicy food.
• If symptoms persist, consult your doctor.

Indigestion:

• Eat smaller and more frequent meals.
• Eat and drink slowly.
• Follow regular physical activity.
• If symptoms persist, consult your doctor.

Diarrhea:

• Stay hydrated with fluids like water, fruit juice, or electrolytes.
• Avoid self-medication and consult your doctor if symptoms persist.

Stomach Pain:

• Rest and relax.
• Eat and drink slowly or have smaller and more frequent meals.
• Apply a heat pad to the stomach.
• Avoid rich or spicy food.
• If symptoms persist, consult your doctor.

Headache:

• Rest and relax.
• Stay hydrated with fluids.
• Apply a pain-relieving balm.
• Avoid excessive alcohol.
• If symptoms persist, consult your doctor.

Pharmacology

• Angiotensin II: A potent vasoconstrictor, a key vasoactive hormone in the renin-angiotensin system, and a crucial element in hypertension pathophysiology.
• Losartan: Blocks the binding of angiotensin II to the AT1 receptor, reducing its vasoconstrictor and aldosterone-secreting effects.
• Hydrochlorothiazide: A thiazide diuretic impacting electrolyte reabsorption in kidney tubules, leading to increased sodium and chloride excretion.

Dosage of Hypertension:

• Initial dose: 50/12.5, one tablet once daily.
• Inadequate response: Increase to 100/25 once daily.
• Inadequate control with Losartan 100 mg monotherapy: Switch to 100/12.5 once daily.
• Hypertensive patients with left ventricular hypertrophy: Initial dose 50/12.5, may increase to 100/12.5 and then to 100/25 if needed. Max dose: 100/25 once daily.
• Antihypertensive effects are usually achieved within three weeks.
• No initial dosage adjustment for elderly patients. Max dose of 100/25 not for initial therapy in the elderly.

Dosage of Severe Hypertension:

• Initial treatment: 50/12.5 once daily.
• Inadequate response after 2 to 4 weeks: Increase to 100/25 once daily. Max dose: 100/25 once daily.

Administration of

May be administered with other antihypertensive agents. Can be taken with or without food.

Interaction:

• Losartan Potassium: No significant pharmacokinetic interactions were found with Hydrochlorothiazide, Digoxin, Warfarin, Cimetidine, and Phenobarbital.
• Concomitant use with potassium-sparing diuretics, potassium supplements, or salt replacements containing potassium may increase serum potassium.
• Interaction with Indomethacin may decrease antihypertensive effects.

Contraindications:

• Contraindicated in patients hypersensitive to either component.
• Contraindicated in patients with anuria or hypersensitivity to other sulfonamide-derived pharmaceuticals (Hydrochlorothiazide).

Pregnancy & Lactation:

• Not recommended during pregnancy.
• Not recommended for nursing mothers.

Precautions & Warnings:

• Hypersensitivity reactions, including angioedema.
• Periodic measurement of serum electrolytes to detect possible imbalance.
• Risk of hypokalemia, especially with brisk diuresis, severe cirrhosis, or prolonged therapy.
• Impaired renal function and symptomatic hypotension.
