Noclog Plus Tablet is a combination of Aspirin and Clopidogrel, classified under Antiplatelets. It is prescribed for preventing early and long-term atherothrombotic events such as heart attack, stroke, cardiovascular death, or refractory ischemia in patients with acute coronary syndrome, with or without cardiac revascularization.

Uses of Noclog Plus Tablet

How does Noclog Plus Tablet work?

Aspirin works by preventing the platelets in your blood from clumping and clotting in your arteries and promoting the smooth flow of blood. Clopidogrel works by blocking platelets from sticking together and preventing them from forming harmful clots. Noclog Plus Tablet is thus effective in reducing the strain on your heart and connected blood vessels and the risk of blood clot formation.

Indications:

Acute Coronary Syndrome (ACS): Clopidogrel is suitable for reducing the occurrence of myocardial infarction (MI) and stroke in patients with non-ST-segment elevation ACS (Unstable angina / non-ST-segment elevation myocardial infarction). It is also used to reduce the incidence of myocardial infarction and stroke in patients with ST-elevation myocardial infarction (STEMI).

Recent MI, Recent Stroke, or Confirmed Peripheral Artery Disease: For patients with confirmed peripheral artery disease or recent myocardial infarction (MI), or a history of a recent stroke, Clopidogrel can reduce the incidence of myocardial infarction and stroke.

Pharmacology

Clopidogrel is a prodrug. It inhibits platelet activation and aggregation by irreversibly binding its active metabolites to P2Y12 ADP receptors on platelets. A dose-dependent inhibition of platelet aggregation can be observed 2 hours after a single oral administration. A repeated daily dose of 75 mg inhibited ADP-induced platelet aggregation on the first day, and the inhibition reached a steady state between the 3rd and the 7th day.

Dosage & Administration:

The recommended oral dose is one tablet daily. Take the Noclog Plus Tablet as advised by your physician. Swallow the Noclog Plus Tablet with a glass of water. Do not crush or chew the medicine. Your doctor will decide the correct dose and duration for you depending on your age, body weight, and disease condition.

Interaction
  • The risk of bleeding is increased by NSAIDs, warfarin, and selective serotonin and norepinephrine reuptake inhibitors (SSRIs, SNRIs).
  • Avoid using Ogilvy Prazole or Esomeprazole at the same time as CYP2C19 inhibitors (omeprazole or esomeprazole). Repaglinide (using Repcomit2C substrate) (using Repcomit2C substrate) raises Repaglinide plasma concentration
Contraindications

Clopidogrel is contraindicated in the following situations: Allergic to any component of the drug or product. Active pathological bleeding, such as a peptic ulcer or intracranial hemorrhage.

Pregnancy & Lactation

There is no adequate and well-controlled study of pregnant women. It should be used during pregnancy only when necessary. It is not known whether clopidogrel is excreted in human milk. The importance of the drug to the mother should be considered and a decision made to stop breastfeeding or discontinue the drug.

Precautions & Warnings
  • Because it is a prodrug, CYP2C19 (poor metabolizer) gene mutations and medications that inhibit CYP2C19 have an impact on how its active metabolites are metabolized (such as omeprazole and esomeprazole).
  • When these medications and CYP2C19 weak metabolizers are taken together, the antiplatelet action of clopidogrel may be decreased.
  • It may make bleeding more likely because it prevents platelet aggregation during the whole life cycle (710 days) of platelets. Platelet transfusion within 4 hours of the loading dosage or 2 hours of the maintenance dose may be less efficient in restoring hemostasis.
  • Clopidogrel discontinuation raises the risk of cardiovascular events.
  • Due to the possibility of significant bleeding, medication should be stopped five days before elective surgery.
  • After hemostasis, clopidogrel was immediately resumed.
  • According to reports, plasma exchange is a necessary component of immediate treatment for thrombotic thrombocytopenic purpura (TTP) (plasma exchange).
  • Allergy, including rash, angioedema, or hematological responses, have been observed in individuals taking clopidogrel or in patients who have a history of allergies to other thienopyridines.
