Pharmacology:

Rivastigmine is a carbamate derivative that shares similarities with physostigmine but differs from donepezil or tacrine in its structure. While the precise mechanism of rivastigmine remains unknown, it is believed to reversibly bind to and deactivate cholinesterase enzymes, including acetylcholinesterase and butyrylcholinesterase. This action inhibits the hydrolysis of acetylcholine, leading to an increase in acetylcholine levels at cholinergic synapses. Notably, rivastigmine's anticholinesterase effects are relatively selective for brain acetylcholinesterase and butyrylcholinesterase compared to peripheral tissues.

Dosage & Administration:

Parkinson Dementia: Transdermal

  • Initial: Apply 4.6 mg patch every 24 hours
  • May increase the dose to 9.5 mg patch every 24 hours after a minimum of 4 weeks if well-tolerated; after an additional 4 weeks, may further increase to 13.3 mg patch if needed

Transdermal in Mild-to-moderate hepatic impairment: Not to exceed 4.6 mg patch every 24 hours

Renal Dose for Transdermal: No dosage adjustment is required

To apply the Transdermal Patch:
  • Choose a suitable area: Select a clean, dry, and healthy skin area without hair. The upper or lower back, upper arm, or chest are recommended areas.
  • Clean the skin: Ensure that the chosen area is clean and free from any creams, oils, lotions, or powder.
  • Avoid damaged skin: Do not apply the patch on damaged or broken skin to prevent irritation.
  • Remove old patch: If you are already using a patch, do not apply a new one until it's time to replace the old patch. Applying a new patch on top of an existing one can lead to overdosing.
