Indication of Epoetin 2000 IU Injection

• Anemia is associated with Chronic Renal Failure (CRF), both in patients on dialysis and those not on dialysis.
• Anemia in patients with non-myeloid malignancies undergoing concomitant chemotherapy.
• Anemic patients (hemoglobin >10 to <13 g/dL) are scheduled for elective, non-cardiac, non-vascular surgery to reduce the need for allogeneic blood transfusions.
• Anemia related to therapy with zidovudine in HIV-infected patients.

Pharmacology

• Epoetin 2000 IU regulates erythropoiesis by stimulating the differentiation and proliferation of erythroid precursors.
• It stimulates the release of reticulocytes into circulation and the synthesis of cellular hemoglobin.
• Recombinant human Epoetin 2000 IU, available as epoetin alfa and epoetin beta, is used in the management of anemias associated with CRF, cancer chemotherapy, and zidovudine therapy in HIV patients.

Dosage & Administration of Epoetin 2000 IU Injection

• The starting dose for adults is usually 50 to 100 IU/kg three times a week (TIW) by IV or subcutaneous route.
• Maintenance dose must be individualized for each patient, typically ranging from 75 to 150 IU/kg/week.
• Administer intravenously over 1-2 minutes.
• Slow injection over 5 minutes may be beneficial for those experiencing flu-like symptoms.
• Do not administer by intravenous infusion or in conjunction with other drug solutions.

Interaction

• Haematinics enhance efficiency.
• Increased dose of heparin in patients undergoing dialysis.

Contraindications

• Uncontrolled hypertension.
• Known hypersensitivity to mammalian cell-derived products.
• History of hypersensitivity to Epoetin 2000 IU or any component of the preparation.

Pregnancy & Lactation

• Pregnancy Category C. Use during pregnancy only if needed.
• Caution is advised during breastfeeding.

Precautions & Warnings

• Caution in patients with controlled hypertension, ischaemic vascular disease, history of seizures, or suspected allergy.
• Iron evaluation before and during therapy.
• Virtually all patients will require supplemental iron.
• Overdose effects: Temporary drug therapy withholding if the hemoglobin exceeds the suggested target range.
