Indication of Diretic-DS Tablet

Diretic-DS Tablet contains Spironolactone, which is a type of Diuretic medication used to treat conditions such as fluid buildup in tissues due to heart disorders, severe heart failure (NYHA III-IV), and certain kidney disorders. It is also prescribed to treat high blood pressure in combination with a salt-free diet and diuretics. Additionally, Diretic-DS Tablet can be used to diagnose and treat Conn's disease, which is a disorder characterized by excess production of aldosterone in the adrenal cortex.

Normally, fluids move into and out of the body's tissues. However, when fluid is not properly removed from tissues, it can cause fluid retention or edema. This condition is characterized by swelling and pain in affected body parts like the feet, ankles, or hands. Diretic-DS tablets can help relieve these symptoms by removing excess fluid from the body.

Pharmacology

Potassium-saving diuretics Spironolactone and Furosemide have different but complementary methods and sites of action. Therefore, when administered jointly, they produce a synergistic diuretic or additional ingredient. The Furosemide component blocks the reabsorption of sodium, potassium, and chloride particles and represses the Na+/K+/2Cl- co-transporter in the ascending Circle of Henle, increasing the amount of sodium and the volume of water expelled in the urine. This inevitably triggers potassium misfortune. By inhibiting the action of aldosterone, the spironolactone component prevents the reabsorption of sodium in exchange for potassium at the distal tubule, favoring sodium excretion and reducing the surplus potassium tragedy caused by furosemide.

Dosage & Administration of Diretic-DS Tablet

• According to the patient's response, administer 1 to 4 tablets each day each containing 20 to 80 mg of furosemide and 50 to 200 mg of spironolactone.
• For patients who have already reached stabilization and need to take their next dosage of spironolactone and furosemide, this tablet contains the active ingredients in the amounts of one to two tablets per day (Furosemide 40 to 80 mg and spironolactone 50 to 100 mg).
• Spironolactone and furosemide use in children: Use of these medications in children is not recommended. Both spironolactone and furosemide may be eliminated more gradually in elderly people.

Take this medicine in the dose and duration as advised by your doctor. Swallow it as a whole. Do not chew, crush or break it. Diretic-DS tablets may be taken with or without food, but it is better to take them at a fixed time.

Interaction

There is a higher risk of hyperkalemia when taken with Expert inhibitors or potassium salts. Spironolactone increases the blood levels of cardiac glycosides like digoxin, which may make digitalis hazardous. If Spironolactone is taken along with corticosteroids, hypokalemia may result. When combined with indomethacin and maybe other non-steroidal anti-inflammatory medicines, the blood weight-lowering and diuretic effects of furosemide may be diminished or eliminated (NSAIDs). Furosemide may make aminoglycoside antimicrobials more harmful to the ears. Sucralfate and Furosemide used together may lessen the natriuretic and anti-hypertensive effects of Furosemide.

Contraindications

Contraindicated in individuals with anuria, severe renal impairment, rapid breakdown, or excessive impedance of renal function (creatinine clearance 30 ml/min), hyperkalemia, Addison's disease, and in patients who are easily harmed by spironolactone, furosemide, or sulphonamides.

Pregnancy & Lactation

• Pregnancy: Spironolactone and its metabolites may pass the placental barrier during pregnancy. The projected benefit of spironolactone use during pregnancy must be evaluated against potential dangers to the mother and developing fetus. Research on animal teratology suggests that furosemide may result in prenatal abnormalities. Furosemide should therefore be used in women of reproductive age as it has in the past when appropriate preventative precautions are adopted or when the possible advantages outweigh the potential risks to the embryo.
• Lactation: The breast drain had Spironolactone metabolites found in it. If Spironolactone use is deemed necessary, organizations should seek an alternative method of infant feeding. Breastfeeding should be discontinued if furosemide medication is necessary because it excretes in breast milk.

Precautions & Warnings

• Diretic-DS Tablet (FRUSEMIDE+SPIRONOLACTONE) is not recommended for people who are allergic to the medication, have low blood pressure, have a history of heart attack or kidney disease with creatinine clearance less than 30 mL/min, have liver disease, or are pregnant or planning to become pregnant.
• It should also not be used by people with low blood pressure, cardiogenic shock, or aortic stenosis.
• Breastfeeding women should inform their doctor before taking Diretic-DS Tablet, as the medication can pass into breast milk and its effect on the baby is unknown.
• Patients who rely on their voice for work, such as actors, singers, or teachers, should be cautious as Diretic-DS Tablet may cause vocal changes.
• Before taking Diretic-DS Tablet, patients with hypovolemia or dehydration and electrolyte and acid-base disturbances should correct these conditions.
• Treatment with Diretic-DS Tablet requires regular monitoring of serum potassium, sodium, and creatinine level.
