How does Diactin 5 work?

It works by increasing the amount of insulin released by the pancreas to lower blood glucose.

The most potential adverse reaction is hypoglycemia (1% to 3.4%). Other reported side effects include asthenia, headache, dizziness, anxiety, tremor, and diarrhea.

Indication of Diactin 5

Glipizide is indicated as an adjunct to diet for the control of hyperglycemia and its associated symptoms in the treatment of non-insulin-dependent diabetes mellitus (NIDDM type II) when dietary modification has not been proven effective on its own. In certain patients receiving insulin, the concurrent use of Glipizide may permit a reduction in the daily dose of insulin. The use of Glipizide should be recognized by both the physician and patient as a treatment in addition to diet and not as a substitute for diet or as a convenient tool for avoiding dietary restraint. Additionally, short-term administration may be required if diet control alone results in temporary control of blood glucose levels. If satisfactory lowering of blood glucose is no longer achieved during maintenance, the use of Glipizide should be discontinued.

Pharmacology

Glipizide lowers blood glucose by stimulating the release of insulin from the pancreas, an effect dependent upon functioning beta cells. The extrapancreatic effects of Glipizide include an increase in peripheral sensitivity and a reduction in hepatic glucose production.

Dosage & Administration of Diactin 5
  • The dosage of Glipizide is not fixed and may be adjusted through periodic monitoring of blood glucose levels.
  • In general, Glipizide should be given approximately 30 minutes before a meal to achieve the maximum reduction in postprandial hyperglycemia.
  • Initial dose: 5 mg before breakfast. Geriatric patients or those with liver disease may be started on 2.5 mg.
  • Dosage adjustments: Done at intervals of several days by an increment of 2.5-5 mg, determined by blood glucose response.
  • Maintenance: Stabilized on a dosage ranging from 2.5 to 30 mg daily. A total daily dose above 30 mg should be divided and given before meals of adequate calorie content.
  • Elderly, debilitated, or malnourished patients: Initial and maintenance dosing should be conservative to avoid hypoglycaemic reactions.
Interaction

The hypoglycaemic activity of sulphonylureas may be potentiated by certain drugs, including non-steroidal anti-inflammatory agents and other highly protein-bound drugs (e.g., salicylates, sulphonamides, chloramphenicol, probenecid, coumarins, monoamine oxidase inhibitors, and β-adrenergic blocking agents).

Contraindications
  • Patients are allergic to Glipizide or any component of the product.
  • Juvenile onset diabetes.
  • Severe or unstable ‘brittle’ diabetes.
  • Diabetes is complicated by ketosis and acidosis, major surgery, serious sepsis, or severe trauma.
  • Severe renal, hepatic, or thyroid impairment, co-existent renal and hepatic disease.
Pregnancy & Lactation
  • Pregnancy: Use during pregnancy should be justified only if the potential benefit outweighs the potential risk to the baby. It should be discontinued at least one month before the expected delivery date.
  • Lactation: Breastfeeding is not recommended while taking this medication.

  • Hypoglycaemia: All sulphonylurea drugs can cause severe hypoglycemia. Proper patient selection, dosage, and education are crucial to avoid hypoglycaemic episodes.
  • Renal or hepatic impairment: This may lead to elevated blood levels of Glipizide and an increased risk of serious hypoglycaemic reactions.
  • Elderly, debilitated, or malnourished patients: Particularly vulnerable to the hypoglycaemic actions of glucose-lowering drugs.
