A vasodilator is amlodipine. It relaxes the blood arteries by preventing the narrowing-causing entrance of specific ions (calcium ions) into them. As a result, blood may move through blood vessels more easily. Together, they ease the heart's workload and enable the organ to pump blood more easily and at lower pressure.

Indication of Arbitel AM 5/80mg Tablet

Arbitel AM 5/80mg Tablet is a combination medicine that consists of Amlodipine and Telmisartan. It is used for the treatment of hypertension (high blood pressure). This medicine relaxes your blood vessels, and heart muscles and decreases your blood pressure. 

Pharmacology

Telmisartan and amlodipine are included in this combo at a set dosage. The non-peptide angiotensin receptor blocker (ARB) telmisartan targets the AT1 subtype of the angiotensin II receptor. The primary stressor of the renin-angiotensin system, angiotensin II causes hypertension (increased blood pressure) by mechanisms such as vasoconstriction, stimulation of aldosterone production and release, cardiac stimulation, and renal reabsorption of salt. Telmisartan selectively prevents angiotensin II from binding to the AT1 receptor in a variety of tissues, including vascular smooth muscle and the adrenal gland, in order to inhibit the vasoconstrictor and aldosterone-lowering actions of angiotensin II. Telmisartan opens up the blood vessels, bringing down blood pressure without slowing the heartbeat. For the AT1 receptor, telmisartan has a substantially greater affinity (> 3000 times that of the AT2 receptor). Other hormone receptors and ion channels known to have a role in the control of the heart are neither bound nor blocked by it.

Amlodipine, a dihydropyridine calcium channel blocker (CCB), prevents calcium ions from exiting via the smooth muscle of the heart and vascular system. Amlodipine is a peripheral vasodilator that works directly on vascular smooth muscle to lower blood pressure and reduces peripheral vascular resistance.

Dosage & Administration of Arbitel AM 5/80mg Tablet

These tablets may be prescribed as an initial therapy if it's unlikely that a patient's blood pressure can be controlled with a single medication. The starting dose is typically 40/5 mg once daily, but patients who require a greater reduction in blood pressure may begin with 80/5 mg once daily. However, this medication should not be used as an initial therapy for patients who are 75 years or older or have hepatic impairment.

In cases where a patient's blood pressure is not sufficiently controlled with either amlodipine or telmisartan alone, these tablets may be used as an add-on therapy. Patients who experience adverse reactions such as edema while taking 10 mg of amlodipine may switch to these tablets once daily at a dose of 40/5 mg, which can reduce the dose of amlodipine without reducing the overall expected reduction in blood pressure.

In cases where a patient is already receiving amlodipine and telmisartan separately, they may instead receive this tablet containing the same component doses once daily as a replacement therapy. The dosage of this tablet must be individualized, and it may be increased after at least two weeks. The maximum recommended dose is 80/10 mg once daily.

Interaction

Other blood pressure medications (ramipril, aliskiren, simvastatin), pain relievers (ibuprofen, naproxen), a medication for heart failure and irregular heartbeats (digoxin), a medication for bipolar disorder (lithium), and immunosuppressants may interact with Arbitel AM 5/80mg Tablet (cyclosporine or tacrolimus).

Reduce your intake of foods high in potassium and stay away from juice, grapefruit, and over-the-counter potassium supplements.

Before using Arbitel AM 5/80mg Tablet, those with diabetes, hyperkalemia (high potassium levels in the blood), low blood pressure, and renal, and liver issues should speak with a doctor.

Contraindications

Arbitel AM 5/80 Tablet Known hypersensitivity to this product or any of its components. Pregnancy & lactation, Biliary obstructive disorders, severe hepatic impairment, hypotension, cardiogenic shock, left ventricle outflow tract obstruction.

Pregnancy & Lactation

Pregnancy: When pregnancy is detected, Arbitel AM 5/80 Tablet should be discontinued as soon as possible. Pregnancy Categories C (first trimester) and D (second and third trimesters).

Lactation: It is not known whether Telmisartan and Amlodipine are excreted in human milk. Because of the potential for adverse effects on the nursing infant, discontinue nursing or discontinue the drug after taking into account the importance of the drug to the moth
